The Supported Living Fund (SLF) is a new type of accommodation support funding for people with a disability aged 18 to 64 years averaging $50,000 per person per annum. It enables people with a disability, along with their family and carers to create living arrangements that suit the person’s life and preferences, gives access to paid supports and services, and to plan for their future. The funding is portable, remains with the individual and is administered by a service provider of the person’s choice.
Applying for the Supported Living Fund
The third and final application round for SLF packages will be open between Monday 24 June and Friday 30 August 2013.
Application process
As with all previous rounds, the SLF Proposal (My Proposal) is the only official application form for Round 3. Regional SLF Assessment Panels will use the information contained in Proposal Forms to assess and prioritise all SLF applicants. No other correspondence or supporting documentation will be taken into account and should not be submitted.
ADHC will be accepting applications from new applicants and people who were unsuccessful in their application for the Supported Living Fund in Round 1 and 2.
